Population Health Management - Big Integrated Data Analytics
Population health management (PHM) is about how we achieve the best holistic outcomes and integrated care at scale inclusively across our communities.
Our first step is to integrate all clinical medical social and wellbeing data for your population, in a way that enables you to best understand which services patients are using, where how and when. This requires population level longitudinal big data and analytics to present the quantum of service utilization at a patient/person level. By bringing this data together with patient level costing data, we then understand how resources – budgets workforce-HHR estates & capital – are utilized across our systems communities and users. In particular, this captures those patients and services which are high intensity, high risk, poor value and reactive/crisis driven. Importantly the data enables us to review which services are delivering best value, the value proposition to invest more or less and the business case to invest in new care and delivery models.
